Are GoodStart Select tests covered by insurance? Most commercial insurance companies provide coverage for our tests, depending on your individual plan benefits.

What are the out-of-pocket costs for testing? My insurance company is requesting your National Provider Identifier (NPI) number and Taxpayer Identification (TIN) number, what are these? My insurance company is requesting CPT codes. What are the CPT codes for GoodStart Select tests? CPT codes are those billing codes used when Good Start Genetics files a claim to your insurance, these codes represent the testing you will be having/have had done.Please see attached pre-verification form for a list of our CPT codes.
I was sent a Good Start Genetics kit and a test order form from my physician.  What do I do next? I received a check from my insurance company.  What do I do with it? Some insurers may send the payment for our services directly to you, in which case you may have received a check and Explanation of Benefits (EOB) from your insurance company for our services.  We ask that you please endorse the check and send it along with a copy of your EOB to Good Start Genetics so that this payment may be applied to your account.To endorse the check please write the words “Pay to the order of Good Start Genetics” in the line typically marked “endorsements” appearing on the back side of the check.  Please sign your name here as well.Mail check and EOB to:
